I don’t maintain a waitlist. Once you decide testing is right for your child, appointments are scheduled promptly. To initiate an evaluation, please register an account for your child using the patient portal link below. Once registered, please complete the new patient forms located under the My Docs and Forms tab. My office will then contact you to schedule the appointments.
